We have a Nurse ( RN ) Medical Manager position in the South Hadley Falls, Ma area with an accredited community health center that has been providing medical and dental care in the area since 1970.
What you'll be doing:
In summary, Nurse ( RN ) Medical Manager works with primary and specialty care medical providers to deliver nursing support to patients living with HIV / AIDS with special emphasis on medication adherence, health promotion, harm reduction, referrals to necessary community recourses, developing treatment plans and facilitating access to state-of-the-art HIV / AIDS care. The RN Medical Manager is part of a regional partnership for HIV / AIDS care funded by the federal Ryan White Title III program (HRSA) and state funds through the Massachusetts Department of Public Health (MDPH). This position reports directly to the HIV / AIDS Services Program Manager.
Other general duties include, but are not limited to, the following:
* Provide direct patient care in the form of skilled nursing visits and educational sessions for HIV patients and their families/support persons
* Develop teaching tools and treatment plans for patients to enhance adherence to complicated medication regimens
* Collaborate closely with treating providers and social case managers to identify patients in need of nursing support and develop plans to provide it
* Manage specifically designated HIV care clinic sessions
* Document all RN visits in the form of SOAP notes, and update all relevant patient information in the medical record
* Bill third party payers for reimbursable RN visits. Minimum requirement of 5 reimbursable visits per week
* Collect and submit all data in accordance with federal and state funder requirements
* Attend regional partnership Clinical Leadership Team meetings
* Work with supervisor to submit all required progress reports
* Serve as an agency representative in the partnership as required
* Perform other related duties as required or assigned
What you need for this position:
* Minimum of two years clinical experience in HIV / AIDS
* Current Massachusetts Nursing Board of Registration License (ACRN preferred)
* Basic computer skills and the ability to use a data tracking program
